Rare Condition Passport

Put the information that matters in your hands

The Rare Condition Passport is a practical tool to help people living with rare conditions share the information they want others to know about their health, communication, care and individual needs.

The Passports are designed to be used in partnership. People and families decide what information they want to share, while health professionals can help complete it, recognise it when it is presented and advocate for its use across healthcare and other relevant settings.

For people & families

Share what you want people to know

Your Passport is about you—not just your diagnosis.

The Rare Condition Passports gives you a practical way to record and share information that matters to you. You decide what you want healthcare professionals, educators and others to know.

Use your Passport to share

  • Information about your rare condition and health
  • What is important to you and your care
  • How you prefer people to communicate with you
  • Information you want healthcare professionals, educators and others to know

For health professionals

Complete it together. Recognise it. Advocate for it.

Health professionals have an important role in making the Passports work in practice.

Work with patients and families to complete their Rare Condition Passport, use the information they choose to share to inform care, and help make the Passport recognised across healthcare settings.

Health professionals can

  • Complete the Passports in partnership with patients and families
  • Recognise and use the Passports when it is presented
  • Encourage colleagues and services to use it
  • Advocate for recognition across healthcare and other relevant settings, including education
